Responsibilities:

Evaluate and provide feedback to direct care staff (home health aide) in the home per

policy.

Provide education and training to the direct care staff while in the client’s home, if

necessary

Audit client charts as needed to ensure aides are following the Plan of Care and

documenting correctly

Assess clients as needed

Perform client medication set-ups, on an ongoing basis, as needed

Provide in-service education topics to RN manager based on deficits of care observed

during the supervisory visits or reported from the clients

Completes all documentation timely and accurately

Available as a clinical resource to staff, as needed. This requires being available by

phone outside normal schedules

Report any concerns/issues to Branch Manager

Maintains confidentiality regarding client information and all related documentation per

HIPPA guidelines

Requirements:

Licensed RN in the state of Michigan

At Least 2 years’ experience in acute or long-term care while employed as a RN

Driver's License

Registered Nurses must comply with state background screening requirements. Compensation,
